Vanderbilt Health Affiliated Network is offering a Project ECHO for community primary care physicians on palliative care. Virtual 60-minute sessions will be held every two weeks for a maximum of 2 sessions. Each session will consist of a didactic portion, case presentation, discussion and recommendations. This will be available to VHAN participating physicians and mid-levels clinicians.


Objectives
After participating in this educational activity, you should be able to:

  1. This series is designed to provide evidence-based information in promoting palliative care in an outpatient setting.
  2. This series seeks to close gaps in palliative care knowledge of primary care physicians.
  3. Identify opportunities in patient care who may benefit from difficult conversations.
  4. To develop an individualized palliative treatment plan to address their complex symptoms.
  5. Participants will become familiar with medication management for patients with complex and terminal illness.
  6. Participants will become familiar with collaboration of care with caregivers and other community partners as needed to coordinate care.
  7. As a result of completing this educational activity, participants will be better able to utilize recommendations from current clinical guidelines.


Vanderbilt University Medical Center is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

Vanderbilt University Medical Center designates this live activity for a maximum 1.00 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.00 Medical Knowledge (Part II) MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn up to 1.00 MOC points in the American Board of Pediatrics’ (ABP) Maintenance of Certification (MOC) program. It is the CME activity provider’s responsibility to submit learner completion information to ACCME for the purpose of granting ABP MOC credit.

PLEASE NOTE: To receive CME or MOC II credit for this activity, the evaluation component must be completed. To receive MOC II credit, the evaluation must be completed no later than November 30th. Evaluations completed after this date will award CME credit only. Evaluations can be found in your CloudCME portal under the Evaluation & Certificates tab.

PLEASE NOTE: This course does not award MOC II credit for the month of December. MOC II credit can only be reported within the same year the credit was earned.
