Project ECHO is a model of regular, interactive, online learning that provides access to professional advice, and aims to support and empower health, education, and social service professionals to practice at the top of their scope supporting people with complex, chronic, and common conditions. The Tennessee Dementia ECHO series aims to support primary care providers working with older adults in the community setting. The health system will soon be overwhelmed as more and more Americans age and many develop dementia. We hope to address this gap by delivering mentoring and advice tailored to you and those you serve. To make this series as valuable as possible, we need to know what gaps exist from your perspective.


Objectives
After participating in this educational activity, you should be able to:

  1. 1. Explain the pathological processes of the most common forms of dementia. 2. Summarize current epidemiological trends of dementia within Tennessee and more broadly the USA. 3. Infer the benefits of proactive screening for dementia as it relates to health outcomes
  2. 1. Recognize common behaviors in people living with dementia 2. Recommend environmental interventions to mitigate behaviors 3. Compare/contrast the risks and benefits of different pharmacologic management strategies (AchEi, SSRI, sedatives, anti-psychotics).
  3. 1. Access and interpret results from prognostic, cancer screening, and surgical risk calculators 2. Appraise when to deprescribe AchEi and dementia-specific medications, 3. Compare and contrast palliative care and hospice eligibility/referral criteria.
  4. 1. Assess and document patient decision-making capacity using validated tools and clinical judgment. 2. Differentiate between conservatorship, guardianship, and power of attorney in the context of primary care decision-making. 3. Formulate care plans that incorporate legal considerations, advance directives, and surrogate decision-maker involvement.
  5. 1. Identify key indicators of changing care needs in individuals living with dementia and their caregivers. 2. Explore available community resources and strategies for effective navigation during transitions in care. 3. Discuss best practices for collaboration among healthcare providers, caregivers, and community organizations to ensure continuity of support.
  6. 1. Describe the current state of biomarkers in neurodegenerative disease 2. Apply biomarker tools in the diagnosis of neurodegenerative disease 3. Compare the specialty care options available in the diagnosis and management of persons with neurodegenerative diseases
