Discussing end of life issues with patients and their families can be challenging. This course will review strategies for initiating and managing these conversations as well as the appropriate documentation for the different elements of advance care planning. The course will also cover how to navigate patient expectations and examine the different roles of hospice versus palliative care.

This course is hosted on VUMC's Learning Exchange.  To begin the course, log in at https://learningexchange.vumc.org and search for Advance Care Planning Module 1.  Once you have completed the content, and passed the post-test with a score of 80% or higher, you will receive a 5 digit text code to claim your credit.



Objectives
After participating in this activity, participants should be able to:

  1. Discuss the importance of engaging a patient in a conversation about Advance Care Planning.
  2. Describe a responsive approach to elicit, document, and follow the preferences of a patient for end of life care.
  3. Discuss how to complete the three forms in advance care planning: appointment of a healthcare surrogate, advance directive for healthcare, physician order for scope of treatment (POST) form and be able to scan these into eStar
  4. Describe the difference between hospice and palliative care and describe the eStar process for hospice referral
  5. Discuss and describe the CMS requirements for reimbursement
